Playmaker Sports Recreational Soccer League Liability Waiver and Privacy Policy

Liability Waiver

I, hereby acknowledge that I have voluntarily chosen to participate in the recreational soccer league organized by Playmaker Sports. In consideration for being allowed to participate in this activity, I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, waive, discharge, and covenant not to sue Playmaker Sports, its officers, employees, volunteers, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or relating to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging to me, while participating in such activity.

I am fully aware of the risks and hazards associated with participation in this activity, including the risk of injury or death. I understand that these risks may be caused by my own actions or inactions, the actions or inactions of others participating in the activity, the conditions in which the activity takes place, or the negligence of the "Releasees" named below. I further understand that there may be other risks not known to me or reasonably foreseeable at this time.

I understand that this waiver and release of liability includes, without limitation, injuries that may occur as a result of equipment malfunction or failure, or any other cause.

I have read and voluntarily signed this Liability Waiver and further agree that no oral representations, statements, or inducement apart from this written agreement have been made.

Privacy Policy

Photo and Video Release

I, grant permission to Playmaker Sports to take and use photographs and/or videos of me during my participation in the recreational soccer league for promotional and advertising purposes, including, but not limited to, sharing on social media platforms, the organization's website, and any other relevant marketing materials.

I understand that these images and/or videos may be used without further notification and that I will not receive any compensation for their use.

Privacy Assurance

Playmaker Sports is committed to protecting your privacy. Any personal information collected during the registration process will be used solely for the purpose of administering the soccer league. This information will not be shared, sold, or disclosed to third parties without your explicit consent, except as required by law.

By participating in the recreational soccer league, you agree to the terms outlined in this Privacy Policy.

Refund Policy

Full refunds will be granted if requested before 2/1/24. After 2/1/24 there will be no refunds.
